Onsite Enrollment Guidelines and Procedure

for School Year 2024-2025

Enrollment Period:  

June 6, 2024 onwards

Admissions & Enrollment One-Stop Shop Venue: 

JHS Mini Theater, 2nd Floor, Administration Building

Campus entrance is at Gate 12, Acacia St., Juna Subd., Matina, Davao City

Office Hours:

Monday – Friday

8:30 am – 11:30 am & 1:00 pm – 4:00 pm

Please observe lunch break.

Saturday

8:30 am – 11:00 am

Enrollment Procedure:

Step 1 – Visit the Info Desk

Step 2 – Proceed to the WTC Desk for the Mental Health Declaration Form

Step 3 – Encoding of Information and Waivers Desk

Step 4 – Payment

Important:

– The presence of the student during enrollment is HIGHLY ENCOURAGED.

– Please wear appropriate attire.

– Single-Use Plastics are not allowed in campus.

2. Educational Service Contracting (ESC) Application and Inquiries

3. Claiming and Requesting of ESC Certificates

4. Returning of Toga

Requirements:

Incoming Grade 7 Students & Incoming Grades 8 to 10 Transfer Students:

  • Digital Copy of Admission Notice of Acceptance (N.O.A.) 

Apply for JHS Admission online at http://jhs.addu.edu.ph/admissions
  • Hard Copy of the following requirements:

1. Official Report Card from Last Grade Level Attended

2. Original PSA Birth Certificate

3. Good Moral Certificate or alternative (ADDU Grade School graduates are exempted from this requirement.)

– Alternative: Recommendation Letter from School Head/Previous Class Adviser or Guidance Counselor

Incoming Grades 8 to 10 Old Students:

  • SY2324 Clearance obligations are complete.

Important:

Special Study Permit or Student Visa for Foreign Students/Students with Non-Philippine Passport shall be processed during enrollment.
